This form should be used by Brigade Captains (or their delegate) to nominate a Brigade team for Navex 08. Should you have any problems with this form, please contact Adam Atkinson on 0447 845101.

Note: Each team needs to be entered on a separate form. Should your Brigade be entering more than one team, you will need to fill in one form for each team.
